We pride in ourselves in creating personalized & friendly experience to deliver excellent service to our patients. Your appointment will be coordinated with your availability in mind and scheduled accordingly.

Once you have your appointment scheduled, it is imperative that Patient Registration be completed prior to your visit to avoid any delays.

​

Initial visit typically consists of obtaining diagnostic imaging and consultation with the doctor with the possibility of same day procedure if needed.

We work with major PPO insurances to provide you the finest care at affordable cost. Every PPO insurances (Such as Metlife, Guardian, Cigna etc.) have different plans and each plan has different benefits. Even the same plans may have different benefits depending on your employer. This is due to your employer who negotiates the plan with insurance companies and administers the plan on your behalf. For this reason, patients are responsible for understanding their plan prior to services rendered.

Initial Consult

Park Avenue Oral Surgery

Your first visit will consist of getting to know you - going over your medical and dental history, physical exam, reviewing any necessary imaging to formulate an accurate diagnosis. Then, we will go over treatment options and answer any of your questions.

Please bring the following if available

  • Referral note

  • X-rays or CT images

  • List of current medications

If additional films are necessary they can be taken at our facility or referred out to an imaging center/hospital

IV Anesthesia

If you are scheduled to have IV anesthesia (Sedation) as part of your procedure

  • Nothing to eat or drink for 6 hours before

  • Adult to take you home 

Recovery

Once completed with surgery, we will discuss with you post-operative instructions in detail with you. If you do not remember - not to worry - you can always refer back to our website under Postop Instructions or reach out to our office. You will also be able to reach the doctor after hours for any questions or concerns.
